Accident Insurance Information

What it does

Pays a cash benefit directly to you if you or your dependents are injured as part of a covered accident, regardless of whether it’s covered by your medical insurance.

How it works

In the event of a covered accident, submit a claim to receive prompt payment. Payment is made directly to you, so you can use it as you see fit.

Short-Term Disability Insurance Information

What it does

Provides a weekly payment, equal to a portion of your salary, that you can use however you choose. Check with your benefits representative for your company’s specific plan details.

How it works

You’ll receive regular payments for a specific period of time once your case has been approved. We’ll just need to receive notice that you are unable to perform your occupation and the supporting medical documents to help prove that. Payments will begin once you’ve been out of work for a set number of days, referred to as your elimination period.

What it doesn’t do

Provide payments for as long as you are disabled and doesn’t provide job protection. This coverage typically provides benefit payments for the first 13 or 26 weeks only, depending on the plan.

Yes. No medical evidence is required for Equitable’s group accident insurance.

If your group accident policy includes a wellness benefit, insureds are eligible to annually receive a benefit amount (default is $50) when they undergo any eligible exam or screening from the covered list.

No. Once you are enrolled and your benefits are effective, there are no waiting periods before benefits can be accessed.

All benefits for Equitable’s group accident insurance are paid directly to you. You can use the payment as you see fit.

Yes. We have two claim forms: one for an accident claim and a short form for the wellness benefit (if included in your plan). The forms are designed to be easy to complete and submit to our claims department. Our average turnaround time for claims is 5 days.

Short-Term Disability Insurance

Short-term disability insurance can help provide peace of mind, because it provides you with replacement of a portion of your income if you become disabled due to an illness, injury, accident or have a baby.

If you have short-term disability insurance and you are disabled under the terms of the contract, you typically start receiving benefit payments after a short elimination period. Those benefits vary based on your disability and can last from several days or weeks up to a maximum period of 26 weeks, depending on the language in your policy. Your benefits generally equal a percentage of your earnings and can be used to pay for any expenses you might have.

You can enroll when you are initially eligible, during any annual enrollment period, or within 31 days of a family status change. Evidence of insurability (also known as proof of good health) may be required. Please see your coverage certificate for details.

To qualify, you must be unable to work due to an illness, injury or accident that is not related to work.

No, you must be totally disabled in order to be eligible for a benefit.

As long as you continue to meet the definition of disability, you can receive benefits for up the maximum benefit period outlined in the “Coverage Details” section. For example, if your benefits commence on the 8th day of disability and you are disabled for 6 weeks, you would receive 5 weeks of benefit payments.

Your Short-Term Disability benefits will be reduced by any state medical leave benefits you may be eligible for.

Maternity claims are treated the same as any other illness

Accident benefits may not be payable for injuries caused or contributed to by or incurred: physical or mental illness or disease or related medical treatment, infection not occurring as a direct result of accidental bodily injury, suicide or intentionally self-inflicted injury, war or act of war, while incarcerated, participating in a felony or illegal activity, intoxication, voluntary drug use unless administered by and used as instructed by a physician or for over-the-counter drugs in accordance with manufacturer’s instructions, participation in certain activities involving an increased risk of injury as listed in the policy and certificate (e.g., mountain climbing, sky diving).

If you start working for your employer after the effective date – the waiting period is determined by your Employer’s personnel policy.

A member who is employed on the effective date of the policy will receive credit toward satisfying the waiting period for time employed with the employer provided he or she was employed on
the day prior to the effective date of the policy.
